Responsibilities:

• Provide one-on-one support to patients during their recovery journey, promoting a healthy and safe environment.

• Assist patients with daily living activities, such as medication management, meal planning, and personal care.

• Monitor and report patient progress to our clinical team, ensuring that individualized treatment plans are being implemented and modified as needed.

• Participate in group therapy sessions, providing emotional support and facilitating patient engagement.

• Maintain accurate and up-to-date records of patient interactions, Progress Notes, and treatment plans.

• Collaborate with our interdisciplinary treatment team to develop and implement patient-centered treatment plans.

• Provide crisis intervention and de-escalation techniques as needed.

• Participate in ongoing training and education to stay current with industry best practices and evidence-based interventions.

Requirements:

• High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
