Position Summary As an Acute Care Pharmacist Specialist at Banner Gateway Medical Center, we are seeking applicants who have completed a PGY2 Pharmacy Residency in Oncology or have equivalent experience working in an oncology clinical pharmacist role. This is an exempt (salaried) position with general day shift hours Monday to Friday and a balanced rotation of some weekend on‑call coverage roughly one weekend every other month. Core Functions Critically evaluate disease states and adjust or recommend adjustments to therapy to improve patient medication regimens; ensure appropriate and cost‑effective medication therapy and document patient assessment data and clinical interventions. Act as a drug information expert in specialty area; provide detailed drug information and education to the healthcare team, pharmacy staff, patients, and the community. Review and evaluate medication orders for appropriateness and completeness, considering patient data such as age, height, weight, diagnosis, allergies, laboratory values, and other pertinent information; comply with directives of the Pharmacy and Therapeutics Committee. Apply continuous quality improvement principles, incorporating quality improvement into other activities and facility/system‑based projects; participate in drug use evaluations and medication safety oversight. Participate in local and system‑based committees; communicate effectively with stakeholders; develop innovative drug‑related patient care practices at institutional and system levels; serve as a resource and mentor to other pharmacists, students, residents, and health‑care providers. Meet ASHP preceptor qualifications and serve as primary preceptor to residents where applicable; may serve as Program Director for the institution’s Pharmacy Practice Residency Program. Perform all functions according to established policies, procedures, regulatory and accreditation requirements, and professional standards; provide Banner Health customers with excellent service by consistently demonstrating core and leader behaviors. Minimum Qualifications A Doctorate in Pharmacy degree and a minimum of one PGY1 residency program. Without residency training, a minimum of three years of health‑system progressive clinical pharmacy experience (evaluating disease state & medication management such as thromboembolic disease, diabetes, oncology, pharmacokinetics, pain management & nutrition, investigational studies, etc.) is required. Additional minimum qualifications include active board certification through the Board of Pharmaceutical Specialties (e.g., BCPS, BCOP). Candidates not currently board certified must achieve certification within 24 months. Pharmacy licensure in the state of practice is required. If not licensed, the applicant must complete the state Board of Pharmacy application process within 30 days of the job offer and be licensed within 90 days of hire. A valid driver’s license and eligibility for company auto insurance coverage may also be required. AZ employees must provide an AZ Board of Pharmacy Wallet Card at time of hire. A current and unrestricted pharmacist license (not intern) is required at the time of hire for BSS applicants. Must possess good organizational, problem‑solving and computer skills; strong written and verbal communication skills for interfacing with staff, physicians, and other contacts; ability to use reference tools to evaluate patient care and answer customer inquiries. Must have knowledge of medication management software, pharmacy system software, Microsoft applications, and automated dispensing systems. Preferred Qualifications Completion of a PGY2 residency or a PGY1 residency plus a minimum of three years of health‑system experience. Board certification in appropriate specialty area. Expert level working knowledge of principles, practices, and operations in assigned area of responsibility. Additional related education and/or experience preferred.
