Family Practice/Primary Care Nurse Practitioner

Responsibilities

• Provide comprehensive primary care services, focusing on both acute and chronic medical conditions such as hypertension, diabetes, and asthma, using integrated clinical pathways.
• Conduct routine physical exams, health screenings, immunizations, and patient counseling with an emphasis on preventative medicine and long-term wellness.
• Collaborate with on-site specialists through the group s extensive internal referral network to ensure coordinated patient care.
• Maintain accurate and timely documentation of patient encounters and care plans within the group s advanced Electronic Health Record (EHR) system.
• Direct and collaborate with a dedicated support team of nurses, medical assistants, and administrative staff to optimize clinical efficiency and patient throughput.
• Participate in clinical workflow optimization and group practice governance, contributing to a structured pathway toward partnership and long-term financial ownership.

Requirements

• Primary Care Experience
• Board Certified Nurse Practitioner
