Part‑Time Registered Nurse (RN) – IRA Group Home, Manhattan, NY Pay rate: $48–$50 per hour Schedule: 14 hours/week, flexible Location: 10016 Manhattan ESSENTIAL RESPONSIBILITIES Coordinate medical care for assigned individuals, monitor and assess health needs, and arrange follow‑up care as needed. Conduct direct observation assessments of consumer health status. Observe signs and symptoms of discomfort, distress, or illness. Evaluate changes in physical status, including exacerbation of conditions, onset of disease, and responses to care; document and follow up on all findings. Communicate with physicians, families, interdisciplinary team members, and clinicians involved in service delivery. Follow up on consumer hospitalizations, emergency‑room visits, injury or trauma reports, and other health issues as required. Accompany individuals to medical appointments when necessary to advocate for and discuss health issues; report findings to the team. Respond to urgent/emergent phone calls outside routine hours regarding individuals on the nurse’s caseload. Attend meetings with management, staff, families, and clinicians to discuss any health‑related issues concerning consumers served. Develop individualized nursing plans and protocols; update and revise these documents as consumer status changes. Supervise AMAP administration of prescribed medication and routine treatments per federal and state regulations and agency policy. Administer STAT medication and treatments as prescribed and participate in in‑services and training of AMAPs. Complete AMAP certification, recertification, and decertification processes as necessary. Review medication administration records (MARs), treatment and adaptive equipment records, and logs of vital signs, elimination, glucose readings, weight, menses, and other specified records; report significant issues to appropriate clinicians and team members. Monitor and manage medication storage and disposal procedures in accordance with federal and state law. Develop and monitor health goals, nursing plans, and protocols that are individualized; review, update, and revise these documents to reflect current consumer status. Monitor consumer dietary and nutritional needs; support staff in managing special meals and feeding of individuals served. Observe meal times for appropriate food choices, portions, and use of adaptive utensils. Participate in re‑admission intake reviews, preparing preliminary plans based on available health records. Participate in discharge planning meetings; write recommendations for referrals to other agencies and follow up on the plan of care. Write individual progress notes and annual nursing assessments/summaries for each individual; submit for physician review and signature as needed.
