Primarily based at home facility may do telehealth for a sister facility nearby. Outpatient neurology lab onsite • Patient Volume: 23 patients per day • Patient Age Group: adults • EMR/Documentation System: Epic • Special Procedures/Unit Details: • Special Equipment Used: EMG EEG Infusion Center Requirements: • Are Board Certifications Req’d?: BC required in Neurology • Which State Licenses Req’d?: SC required • Certifications Req’d? BLS / ACLS / etc: BLS ACLS • Specific Skills Req’d?: • CSR Req’d?: Yes • DEA Req’d?: Yes • Minimum Experience Req’d # of years: 2 years minimum experience • Are vaccines mandatory?: Flu MMR Varicella waivers are accepted • Any additional requirements/details to screen for?: clean backgrounds only if not clean then provider msust have 1st and 3rd party statements included with submission. Credentialing/Privileging Timeframe: 90-120 days with active SC license. Without active SC license 120 minimum recent experience has taken closer to 5 months for non-licensed providers to be granted privileges.
