What You'll Do:

• Screen, evaluate, and develop individualized plans of care for patients with physical functioning disorders and wounds
• Administer manual therapy, therapeutic exercises, modalities, and wound care as part of rehabilitation
• Monitor and document patient progress and adjust treatments accordingly
• Provide consultative support and counseling to patients, families, and caregivers
• Communicate regularly with healthcare team members about patient status and treatment plans
• Complete discharge summaries and maintain accurate, regulatory-compliant documentation
• Attend team meetings, training, and in-services as required
• Support supervising physical therapist assistants, aides, and students as needed
• Comply with all regulatory, ethical, and professional standards

What You Need:

• Current Physical Therapist license in Oklahoma
• At least 1 year of relevant experience, preferably in LTC
• Strong clinical assessment and documentation skills
• Excellent communication and teamwork skills
• Ability to work independently and support patient-centered care
