What You'll Do

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and wound care needs
• Develop and implement individualized care plans following regulatory and clinical standards
• Perform therapeutic procedures, including manual techniques, ambulation, exercises, modalities, supportive devices, and wound care
• Monitor patient progress and adjust treatments accordingly
• Document treatment outcomes and discharge summaries comprehensively
• Collaborate with healthcare team members and communicate patient progress effectively
• Support supervision of physical therapist assistants, aides, or students
• Participate in meetings, trainings, and required conferences
• Obtain necessary supplies and equipment as per clinical guidelines

What You Need

• Valid Physical Therapist license in Oklahoma
• Minimum 1 year of related experience; LTC experience preferred
• Strong communication and patient care skills
• Knowledge of regulatory and clinical documentation standards
• Ability to work effectively in a team environment
