In-Home Care Coordinator & Resource Advocate
Central Ohio Area Agency on Aging
Central Ohio Area Agency on Aging is seeking a Care Coordinator to assess client needs and coordinate services across home care, transportation, meals, housing, and medical appointments in Columbus, OH. You will develop individualized care plans, conduct in-home assessments, and advocate for clients in collaboration with families and providers.
