The RN is responsible for resident assessments, medication administration, clinical decision-making, and collaborating with physicians and the interdisciplinary team to ensure exceptional resident outcomes.ResponsibilitiesProvide skilled nursing care in accordance with physician orders and individualized care plansPerform resident assessments and identify changes in conditionAdminister medications, treatments, IV therapy, and wound care as orderedSupervise and support LPNs and CNAs during the shiftRespond quickly to resident needs and emergenciesComplete accurate and timely clinical documentationCommunicate with physicians regarding changes in resident conditionCoordinate admissions, transfers, and discharges as neededEducate residents and families regarding care and treatment plansFollow infection prevention and infection control protocolsParticipate in Quality Assurance and Performance Improvement (QAPI) initiativesEnsure compliance with CMS, state, and facility regulationsFoster a positive, respectful, and team-oriented work environmentQualificationsCurrent Missouri Registered Nurse (RN) license in good standingCurrent BLS/CPR CertificationSkilled nursing, rehabilitation, long-term care, hospital, or post-acute care experience preferredStrong assessment, critical thinking, and leadership skillsExcellent communication and teamwork abilitiesCompassionate, dependable, and resident-focusedWe Welcome Nurses With Experience InSkilled NursingLong-Term CareRehabilitationPost-Acute CareHospital NursingMedical-Surgical NursingCharge NurseGeriatric NursingWhy Heritage Wellness & Rehabilitation?At Heritage Wellness & Rehabilitation, you'll be part of a team that values clinical excellence, teamwork, and compassionate care.
