Job Description:
We are seeking a dedicated Physical Therapist for a temporary part-time position in home health care. The ideal candidate will design and implement treatment programs aimed at restoring and enhancing patients' physical, social, and mental functions, working collaboratively within an interdisciplinary team. Responsibilities include conducting evaluations to maintain quality standards, applying clinical techniques and tools to develop individualized treatment plans, and providing therapy interventions using established protocols. The therapist will establish personalized care and discharge plans, ensure clear communication with patients and team members, and maintain comprehensive clinical documentation within deadlines. Supervisory duties include overseeing assigned assistants. Additional job-related responsibilities will be assigned as needed.


Hourly Pay Rate: $58.29
