Registered Nurse – Home Health

Schedule: 8 hrs

Assignment Details:

• Contract Weeks: 22
• Guaranteed Hours: 40

Requirements:

• Current state license as a Registered Nurse
• Homecare Homebase Software Experience
• OASIS Experience
• CHPN certification (Preferred)
• Proof of current CPR certification
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years Home health experience
• Current driver's license
• Reliable transportation with insurance

Roles & Responsibilities:

• Assess and respond to the physical, psychosocial, and environmental needs of patients in their homes.
• Implement, document, and evaluate the Plan of Care (POC) to ensure continuity of care.
• Educate patients and families on care protocols, medications, and safety practices.
• Supervise home health aides and provide clinical direction to ensure quality service delivery.
• Perform on-call duties and provide support to patients and families as needed.
• Collaborate effectively with interdisciplinary team members and healthcare professionals.

Compliance / Notes:

• Must meet mandatory continuing education requirements.
• Ability to maintain confidentiality.
• Flexible schedule and local travel may be required.
