SUMMARY: Provides case management and direct patient care within a team environment. Administers comprehensive skilled care.

 

Schedule: Monday - Friday, 8 to 430pm No weekends, no holidays, No on calls

Location: Seneca County

 

Qualifications

Education & Experience - Applicants must possess the following qualifications or an equivalent combination of education and experience:

• Education: Graduate of an approved Occupational Therapy program.
• Experience: Minimum of one-year experience in a hospital inpatient setting.
• Licensure: Possession of current OT license to practice in New York State.

 

Responsibilities

• Provide skilled OT services to patients in their homes.
• Complete patient evaluations and develop individualized treatment and discharge plans.
• Deliver interventions to improve independence, safety, self-care, and daily living skills.
• Educate patients, families, and caregivers on treatment plans, adaptive strategies, and home safety.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with an interdisciplinary care team to coordinate patient-centered care.
• Supervise COTAs as applicable.
• Maintain timely, accurate clinical documentation.
• Communicate changes in patient condition and coordinate appropriate follow-up.
• Connect patients with community resources and support services.
• Participate in care conferences, education, quality improvement, and team initiatives.
• Maintain required productivity, quality, and regulatory standards.

 

Salary: $67,700 - $115,045.00
