Provide ongoing evaluation of patient status and goals, modifying the treatment program for timely progress. Develop comprehensive goals for patient treatment to maximize outcomes and return to community activities. Coordinate treatment programs with other clinical/therapeutic services and available community resources, attending appropriate clinical meetings to facilitate coordination. Educate patients, families, and caregivers, including developing progressive home programs with instruction for a safe and effective discharge. Prepare for patient discharge, including ordering necessary equipment and recommending therapeutic services to ensure continuity of care. Evaluate patients in actual or simulated home, school, work, or community environments as needed to facilitate a return to community activities, making recommendations for necessary adaptations or training. Identify and manage various levels of care necessary to ensure plan delivery, delegating and supervising PTAs and support personnel to maximize outcomes and ensure effective utilization of services. Minimum Qualifications Current state licensure or eligibility for state licensure as a Physical Therapist.
