Carease Health - MI is dedicated to providing comprehensive preventative care programs that prioritize the physical and mental well-being of our patients. We are seeking a compassionate and dedicated Mental Health Provider to join our Social Work/Care Companion team. As a Social Work/Care Companion, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

Job Summary:

As a Social Work/Care Companion, you will be an integral part of our Collaborative Care team, working closely with psychiatric professionals to provide comprehensive care to our patients. You will conduct assessments, implement short-term therapy and supportive interventions, and document visit notes and records. With your strong communication, documentation, and relationship-building skills, you will help our patients feel heard, supported, and emotionally connected within their environment.

Key Responsibilities:

• Visit designated facilities in accordance with the schedule set with your supervisor

• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs

• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns

• Document visit notes and maintain detailed records of interactions and interventions after each visit

• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

Qualifications:

• Associate Degree in Social Work, Psychology, Counseling, or another mental health–related field

• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings

• Strong communication, documentation, and relationship-building skills

• 1 year of experience in a mental health setting
