Detail-oriented HIM Medical Records Clerk responsible for preparing, scanning, and accurately indexing inpatient and outpatient medical records into the electronic health record system. Ensures the integrity and quality of patient documentation by performing thorough quality checks, resolving image issues, and maintaining correct document order, types, and encounters. Supports daily workflow through efficient chart preparation, timely scanning, and adherence to productivity and accuracy standards. Demonstrates strong organizational skills, maintains strict patient confidentiality, communicates issues promptly, and assists with additional duties such as chart pickup and workflow coverage as needed. Detail-oriented HIM Medical Records Clerk responsible for preparing, scanning, and accurately indexing inpatient and outpatient medical records into the electronic health record system. Ensures the integrity and quality of patient documentation by performing thorough quality checks, resolving image issues, and maintaining correct document order, types, and encounters. Supports daily workflow through efficient chart preparation, timely scanning, and adherence to productivity and accuracy standards. Demonstrates strong organizational skills, maintains strict patient confidentiality, communicates issues promptly, and assists with additional duties such as chart pickup and workflow coverage as needed. Position Responsibilities
- Prepares both inpatient and outpatient discharged records to be scanned and indexes into the Horizon Patient Folder (HPF) on a daily basis.
- Is responsible for maintaining the integrity of patient information by accurate and timely indexing/scanning documents into the electronic health records and performs quality checks to ensure documents are being scanned accurately.
- Scans all documents through the Bell & Howell scanner into the Optical Imaging System and re-scans documents when necessary to ensure clarity and accuracy.
- Maintains daily workflow, insuring that records are scanned in a timely and efficient manner to avoid delay in further processing.
- Verifies the electronic document against th paper document to ensure correctness. Performs indexing process with accuracy by ensuring documents are indexed to the correct patient, correct encounter and document name. Assigns each document to the correct document type and verify that bar-coded documents are correctly indexed.
- Verify the quality of images as readable by comparing the document on the computer terminal to the original to ensure accurate indexing.
- Identifies poor quality documents and marks them for rescanning.
- Ensures that all scanned documents are positioned correctly.
- Identifies when it is appropriate to split and merge documents. Performs the splits and merges correctly. Appends and inserts pages/documents when and where appropriate.
- Writes each indexed batch to the appropriate queue according to workflow procedure.
- Maintains expected productivity and quality standards.
- Depending on the department need and by management direction, assist in chart pick up and scanning.
- Prepares all documents for scanning by removing staples, paper clips, sticky notes, and rubber bands. Prepares all documents in date sequence from admission to date of discharge. Groups like document types together, places in page order and in date order correctly 100% of the time.
- Communicates any issues/concerns to the Team Leader or manager with clear detail.
- Assembles all documents in appropriate order as outlined in chart prep policy and in correct order at least 100% of time. Ensures that all charts are assembled within required time frame at least 100% of the time.
- Maintains expected productivity and quality standards.
- Possess strong organizational, time management and prioritization skills
- Maintains patient confidentiality by not revealing and or discussing patient information with anyone who does not have a legitimate interest in the information.
- Willing to work days or shifts other than the one assigned in order to assist the department management with appropriate coverage of workflow.
