Your Impact as a Hospice RN Case Manager

• Develop and manage individualized Plans of Care with patients, families, physicians, and the IDG
• Coordinate all hospice services and ensure seamless care across settings
• Provide expert pain and symptom management
• Educate patients and families on the Medicare Hospice Benefit and hands-on care
• Supervise Home Health Aides and deliver high-quality care
• Complete timely documentation and participate in admissions

What You'll Bring

• Current Oklahoma Registered Nurse license
• At least 1 year of recent medical/surgical, oncology, home health, or hospice experience
• Strong knowledge of pain and symptom management in terminally ill patients
• Excellent communication, organization, and problem-solving skills
• Valid driver's license, reliable transportation, and current auto insurance
