The focus is on the creation of a caring environment that promotes healing and wellness. Nursing practices are based on evidence, current research, and regulatory standards. Clinical decision-making incorporates all four phases of the nursing process. Critical thinking and creativity are essential components of effective nursing practice. Nursing excellence is promoted through collaborative relationships among nursing staff and other disciplines. The RN coordinates and provides direct patient care activities on the unit. This includes nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. Provision of patient and family-centered education is key. Care coordination and individualized treatment planning are performed as applicable. Patient needs are assessed comprehensively in accordance with RN scope of practice. Cultural, age, and developmental factors are taken into account during assessments. Patient risk factors and presenting complaints are identified. Health information is obtained from patients, family, or others as necessary. Both objective and subjective data are collected and documented. Initial admission assessments are verified if performed by other healthcare team members. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions are designed relevant to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family. Patient care therapies and procedures are fully implemented within scope of practice. Procedures and treatments are explained to patients and families. Actions are initiated to alleviate patient stress and discomfort. Changes in patient condition are recognized and communicated appropriately. Ongoing assessments are conducted and documented according to established standards. Tasks are delegated appropriately, and the performance of team members is overseen. The accuracy and completeness of delegated patient care are monitored. Educational needs of patients are identified and addressed. The discharge process is facilitated by executing discharge orders in a timely manner. Patient discharge teaching is performed in coordination with case management. Input into patient/nurse assignments is provided, considering staffing and patient acuity. Active participation in unit-based activities that support organizational values is expected. A positive group dynamic and healing environment are promoted. Nursing excellence is assured through adherence to accepted standards and evidence-based practices. Participation in department and unit quality improvement activities is encouraged. A holistic perspective regarding patient care is utilized.
