Work Environment\n

This position is primarily remote, providing flexibility to work from home while facilitating crucial support services for our clients and their families. However, you will also play a vital role in the hospice center, communicating regularly with team members, attending remote meetings, and potentially visiting clients if necessary. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of patients to determine their social, emotional, and psychological needs.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP) to align treatment with the unique needs of each family.

• Monitor and analyze patient progress as part of a multidisciplinary team, ensuring compliance with all relevant State and Federal regulations.

• Build strong rapport with clients, their families, and team members to foster a supportive and nurturing environment for growth and healing.

• Develop personalized intervention strategies using evidence-based practices tailored to family goals and preferences, all while employing a coaching model.

• Provide continual education, resources, and emotional support to families to enhance their child's development, reinforcing positive parenting techniques.

• Stay informed of and comply with health and safety standards to protect patients and staff within our care environment.

Patient Population / Client Focus\n

The LCSW will primarily support young children experiencing disabilities or developmental delays, along with their families. Your role will be critical in addressing the diverse needs of these families, ensuring they have appropriate resources, emotional stability, and developmental support during a critical period in their lives. Understanding the complexities of childhood development and family dynamics will be essential in this role, allowing you to deliver impactful interventions.

Qualifications\n

• A Master's Degree or Doctorate in Social Work is required.

• Current licensure as an LCSW by the Department of Public Health.

Compensation and Benefits\n
