Housing Case Manager I — Help Clients Secure Permanent Housing

St Joseph Center

St. Joseph Center is seeking a Case Manager I to deliver quality housing support services within the Housing Supportive Services Program. You will provide individualized case management, linkage to health services, and housing placement support in a multi-disciplinary team.
