Heartford Health is a new medical program that starts with a simple idea that care should come to the resident. Our clinicians visit residents in the Senior Living communities where they live, get to know them, and keep each resident's own doctor in the loop after every visit. More access. Greater continuity. Care that is actually connected. We launch this fall in five communities across San Diego County, and we’re looking for the person who will make it feel personal.

Salary range: $80,000 - $95,000

This is a field-based position serving five senior living communities across Escondido, Carlsbad, Pacific Beach, and La Jolla.

Senior Living experience is preferred.

The Resident Navigator is the human face of Heartford Health. You are the one a family calls. You know every resident in the program, you make sure nothing falls through the cracks, and when something goes wrong, you are the one who puts it right. Our clinicians provide the care. You make sure it all fits together for the resident and the people who love them.

You will also be the first part of the program to arrive in each community, weeks before the first clinical visit, meeting the team, meeting residents and families, and explaining what is coming and why it matters. This role does not exist yet. You will be the one who shapes it!

What you will do:

• Be the single point of contact for residents and families — the number they call with a question, a worry, or a “what happens next.”
• Introduce Heartford Health to residents and families and help them join the program. Clear, honest conversations about what it offers and how it works with the care they already have. There are no sales targets here. Once a resident joins, you stay with them — their quarterback for care.
• Keep the clinical visits running smoothly. Coordinate the schedule with our clinicians, make sure each resident is ready, and follow up with the family afterward so everyone knows what happened and what comes next.
• Turn decisions into action. When a clinician recommends a specialist appointment, transport, a piece of equipment or a pharmacy change, you make it happen — and you make sure the results find their way back to the people who need them.
• Be a familiar face in every community you cover, working alongside each community's leadership and care team as a trusted partner.
• Keep families informed, join family meetings when it helps, and own the follow-through when something has not gone the way it should.
• Help us get better. Track how families experience the program and tell us plainly what is working and what is not.

Who you are:

• You take ownership. You see what needs doing and you do it — without waiting to be told. This role comes with a lot of freedom and very little hand-holding, and that sounds like an opportunity to you, not a worry.
• You are warm and steady with people who are anxious. You explain things in plain language, you say what you are going to do, and then you do it.
• You are organized. You can keep dozens of threads moving across five communities without letting one drop.
• You know your lane. You are not a clinician, and you know when — and how quickly — to bring one in.
• You are curious. Senior living has its own rhythm and its own language. If you have not worked in it before, you are eager to learn fast from the people who have.
• You have done this kind of work somewhere. Senior living, healthcare, care coordination, patient advocacy, hospitality — any role where you were responsible for a real outcome for real people. The setting matters less to us than the track record.

Requirements:

• At least 18 years old, and able to pass a criminal background check and health screening — standard for anyone working with residents in a licensed senior living community.
• A valid California driver's license and reliable transportation for travel between the five communities.
• Comfortable and patient with older adults, including people living with dementia.
• Handles residents' personal and health information with care and in line with HIPAA.
• Confident with everyday office software and quick to pick up new systems, including electronic health records.

Heartford Health changes that. Our clinicians come to the resident. They visit regularly, know each resident's story, send a report to the resident's own physician after every visit, and are on call when something happens after hours.
