Unit Care Coordinator

Life Care Centers is seeking a Unit Care Coordinator RN to lead and coordinate patient care on our skilled nursing unit. In this role, you will oversee clinical care plans, assess resident needs, administer medications, and ensure regulatory compliance. You will collaborate closely with an interdisciplinary team, support CNAs and nurses, and communicate with families regarding care goals and progress. This position offers a family-focused, patient-first culture with strong support for professional growth and a commitment to excellent healthcare outcomes for seniors.
