Overview Licensed Practical Nurse (LPN) – Long Term Care, Memory Care – Gunnison, CO. Travel temporary assignment starting 08/24/2026 for 13 weeks. Guaranteed hours 36 per week. Shift: 12-hour day shifts (6:00 a.m. to 6:00 p.m.), three days per week. Weekly pay range: $1,283–$1,346 per week. Work setting focuses on memory care within a long-term care environment. Key Highlights Weekly pay: $1,283–$1,346 per week Guaranteed hours: 36 per week Contract duration: 13 weeks Shift: 12-hour day shifts (6:00 a.m.–6:00 p.m.), three days per week Start date: 08/24/2026 Setting: Long-term care memory care facility in Gunnison, CO Travel assignment with temporary status Responsibilities Provide direct nursing care and supervise day-to-day activities for residents during the assigned shift, in accordance with applicable laws and facility protocols Administer medications, monitor responses, and manage IV therapy where certified; document all pertinent information per policy Implement rehabilitative and restorative nursing interventions as part of residents’ plans of care Maintain accurate documentation and communicate with the Director of Nursing and interdisciplinary team to ensure continuity of care Promote resident safety, comfort, and dignity; respond to emergencies, assessments, and call bells in a timely manner; uphold infection control standards Requirements Active Colorado LPN license Minimum 3 years of experience in a hospital, skilled nursing facility, or comparable setting; training in rehabilitative/restorative nursing Current BLS certification; IV certification preferred Ability to pass Colorado adult protective services (CAPS) check LPN education: Diploma or equivalent Ability to work in a memory-care focused LTC setting Reliable transportation required due to rural location (Gunnison area has limited public transit) Benefits and Support Dedicated support throughout the assignment 24/7 recruiter and travel support Credentialing and licensing assistance Assignment extension opportunities About Healthforce Healthforce is a nationwide travel healthcare staffing agency connecting travel nurses and allied health professionals with high-quality contract assignments across the United States.
