PruittHealth is committed to nondiscrimination and encourages candidates from all backgrounds to learn more at Role Description The Hospice Case Manager is a full-time, on-site role based in Myrtle Beach, SC, responsible for coordinating and delivering comprehensive hospice care to patients and their families. This role oversees individualized care plans, conducts patient assessments, and collaborates with interdisciplinary team members to ensure physical, emotional, and psychosocial needs are met. Day-to-day responsibilities include managing caseloads, monitoring symptom management, educating patients and caregivers about hospice services, and documenting care in accordance with regulatory and organizational standards. The Hospice Case Manager also participates in care conferences, communicates regularly with physicians and other healthcare professionals, and advocates for patient preferences and goals of care. Qualifications Clinical hospice care skills, including patient assessment, care planning, symptom management, and end-of-life support. Strong case management abilities, such as caseload coordination, care transitions, and collaboration with interdisciplinary teams. Effective communication and interpersonal skills to work with patients, families, and healthcare professionals in sensitive situations. Organizational and documentation skills to maintain accurate records and comply with hospice, state, and federal regulations. Active Registered Nurse (RN) license in the state of South Carolina; hospice or palliative care certification is preferred. Previous experience in hospice, home health, or related clinical settings is highly beneficial. Reliable transportation and ability to travel locally to patient residences or facilities as needed.
