Inpatient Case Manager MSW - Discharge & Support

Methodist Hospital Northeast is seeking a Social Worker (MSW) to perform non-clinical biopsychosocial assessments and interventions for inpatient floors and the outpatient ED. The MSW will assist with discharge planning, crisis intervention, and referrals, coordinating with care teams to address financial, emotional and psychosocial needs. The role encompasses work across ICUs, Trauma, Oncology, NICU, Transplant and ED, with opportunities to support patients and families through difficult #J-18808-Ljbffr Joinmethodist
