About This Role As a Field-Based RN Case Manager with UnitedHealth Group, you will support a diverse member population by providing education, advocacy, and connections to necessary resources. This role involves visiting members in the hospital before discharge and assisting them with safe transitions from care environments to their homes. Highlights Pay : $60,200 - $107,400/year Location: Fort Worth, TX Schedule: Full Time, Day shift Travel: Up to a 50-mile radius from home What You'll Do Visit members in the hospital prior to discharge Provide a complete continuum of quality care through close communication with members via in-person or phone interaction Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels Assist members with the transition from a care facility back to their home Requirements Education Current, unrestricted RN license in the State of Texas Experience 3+ years clinical experience in a hospital, acute care, home health/hospice, direct care, or case management Licenses & Certifications Current RN License in Texas required About UnitedHealth Group UnitedHealth Group is a leading healthcare provider committed to delivering exceptional patient care.
