About This Role As an RN Hospice Coordinator at Cleveland Clinic, you will support patients transitioning from hospital to home or a facility, ensuring a smooth adjustment and continuity of care. Highlights Pay: $30.41 - $46.38/hour Location: Independence, OH Facility: Cleveland Clinic Schedule: Full-time, Day shift What You’ll Do Act as a member of the interdisciplinary team for coordination of services. Process inpatient referrals into hospice and coordinate discharge planning. Educate potential customers about the benefits of hospice care. Ensure patients and families understand the hospice benefit. Process inquiries for all new referrals. Ensure seamless transition from hospital to community by ordering medical supplies. Perform assessments of potential patients alongside the physician. Plan delivery of patient care in collaboration with other disciplines. Assess patient for safety risks, abuse, and neglect. Document changes in patient condition and effectiveness of care. Requirements Education Graduate from an accredited school of Professional Nursing Bachelor’s of Science in Nursing (BSN) preferred Experience One year of current clinic experience as an RN One year of experience in an acute med/surg or critical care setting Licenses & Certifications Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification through AHA or American Red Cross About Cleveland Clinic Cleveland Clinic is a leading healthcare provider recognized as one of the top hospitals in the nation.
