Home-Visit Case Manager for Mental Health
Community Health Alliance - Transitional Living, TLC
Community Health Alliance is seeking a compassionate Case Manager to provide direct support to individuals living with mental health conditions by coordinating care, teaching daily living skills and fostering recovery through individualized services. This role is community based and includes home visits, service coordination and collaboration with providers, families and other supports. In this community-based role you will coordinate assessments, treatment planning and crisis intervention, #J-18808-Ljbffr Community Health Alliance - Transitional Living, TLC
