Community Liaison - Home Health (Sales Representative/Account Executive)

Overview Description Full-time role with HealthFlex, a home health and hospice provider serving the San Francisco Bay Area, North Bay, Central Valley, and the Greater Sacramento Area. Role Summary As a Community Liaison, you will serve as a liaison between community partners (including hospitals, physician practices, skilled nursing facilities, assisted living communities) and patients and their families. You will analyze market trends, identify potential partners, and foster relationships to contribute to HealthFlex’s market leadership and delivery of high-quality home health and hospice services. Key Responsibilities Sales Target Achievement: meet and exceed sales targets by promoting HealthFlex’s services and expanding our client base. Market Analysis: monitor market trends in your territory to inform growth strategy. Prospect Identification: identify potential clients and referral sources, including healthcare providers and facilities. Building Business Relationships: establish and foster meaningful business relationships to secure referrals and partnerships. Sales Techniques: utilize a range of sales techniques, from cold-calling to in-service presentations and traditional marketing methods; training is provided. Communication: maintain ongoing, timely communication with internal and external partners to ensure smooth transitions into services and ongoing quality of care.
