Mobile Care Manager

Description Job Overview: This position carries the responsibilities of a Care Manager, with a primary focus on managing a rolling caseload. A rolling caseload provides temporary coverage to ensure families continue receiving consistent support during transitions. These transitions may occur for a variety of reasons, such as staff leave, scheduling adjustments, or when families have urgent needs requiring immediate attention. This role is designed to ensure continuity of care and responsiveness during these transitions Requirements Essential Responsibilities:
- Reaching out within 1 business day to families to make introductions upon receiving new individuals and maintaining
an excellent rapport and relationship - Complete updates on caseload in preparation for reassignment
- Working to improve family satisfaction with services as necessary
- Immediately review charts to check for quality and compliance
- Taking potentially non-compliant cases and bringing them to compliance before redistribution
- Completing a warm transfer to the long-term Care Manager once the case is compliant and a new CM has been
identified.
- Excellent interpersonal skills
- Advanced ability to effectively communicate in both verbal and written manner
- Computer software skills
- Ability to organize, schedule, and utilize time well
- Capability to analyze situations accurately and take effective action
- A Bachelor's degree with two years of relevant experience, OR
- A License as a Registered Nurse with two years or relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
- A Master's degree with one year of relevant experience
- MSC Service Coordinators prior to July 1, 2018 are "grandfathered" to facilitate continuity of care
- Previous experience working at a Care Coordination Organization (CCO) is required
