Reentry Case Manager

Responsibilities Conduct Participant Intake & Assessment Develop Individualized Service Plans (ISPs) Provide Case Management & Support Facilitate Resource Navigation Track Progress & Maintain Documentation Engage in Crisis Intervention Collaborate with Partners Advocate for Participants Support Program Development Requirements Prior experience in case management, social work, re-entry services, or a related field Deeply committed to the dignity, rights, and empowerment of justice-involved individuals Highly organized and able to manage multiple caseloads while maintaining empathy and professionalism Strong verbal and written communication skills Familiar with Washington, D.C.-based resources and systems relevant to returning citizens Trauma-informed and culturally responsive in your approach to client engagement Comfortable using case management databases and common software (Microsoft Office, Google Suite) Bilingual candidates are encouraged to apply #J-18808-Ljbffr
