We have an opportunity to hire a full-time Care Coordinator in our local Fargo, ND Psychiatric Residential Treatment Facility! Position Summary The Care Coordinator is responsible for participating with the therapeutic team while supporting and reinforcing treatment services by providing guidance and direction to assigned individuals, families, and groups, and coordinating internal and external resources to ensure ongoing therapeutic services and community support is delivered for complete treatment objectives for stabilization achievement. Service provision will meet or expected minimum standard. Manages and coordinates resources and activities for each assigned client(s) to ensure that the unique treatment needs, and requirements of each individual are met. Coordinates, provides transportation and facilitates/co-facilitates case conferences and client-oriented team meetings with both internal and external resources including county or state referral sources, court appearances, scheduled family visits, etc. Actively participates in the implementation of individual treatment plans, compiles resident assessments materials, maintains required clinical documentation and reports within provided deadlines. Collaborates with and maintains regular contact with family members, licensing, parole officers, the court system, and other organizations to ensure ongoing consensus on therapeutic treatment direction, behavior expectations, progress, and outcomes. Accurately maintains individual accounts and other residential records as required and ensure basic needs of clients are met (i.e., adequate clothing). Engages youth in the management of their own budgets. Maintains a schedule that is adjustable and flexible to meet changing work needs and demands of clients and their families. Participates and facilitates the client and family’s linkage to the therapeutic treatment process and activities, identifies barriers and connects with support throughout the treatment process to support to post transition or discharge placement. Works closely with the teams to create supported engagement, guidance plans, stabilization plans, treatment plans, therapeutic interventions, and discharge plans. Participates as an active member of an interdisciplinary team to develop treatment plans and completes other client or case documents as needed. Provides assistance for families regarding locating resources available to the families and client that will support the healthy functioning of the family unit. Works to bridge relationships with clients, adults, and families, with outside organizations and provides role modeling for advocating for needs. Participates in and/or facilitates all required meetings and conferences relative to assigned clients. Maintains and completes paperwork and compiles documentation required by the organization and for the governing/accrediting agencies; maintain current files on all individuals enrolled in services. Provides consultation and mental health services to youth, families and support systems and documents provision of services consistent with Medicaid requirements that facilitates the treatment process. Ensures completion of all required documentation standards including, but not limited to, critical incident reporting. Produces informal and formal reports as requested by outside agencies, parents, referral agencies, courts, etc. Works effectively with internal stakeholders to successfully implement plans for achievement of treatment objectives and community reintegration. Participates in the training and development of other care coordinators and assumes mentorship role as assigned. Maintains appropriate boundaries in all interactions and contact with residents. Complies with Nexus policies involving privacy and confidentiality for residents and their families. Demonstrates the ability to handle emergency or crisis situations and use sound judgment by taking appropriate actions. Remains calm and effectively manages stressful workload conditions and difficult, emotionally charged or crisis situations. Demonstrates concern and empathy towards others as needed, while maintaining appropriate boundaries. Exhibits a high degree of ethical conduct, integrity, and confidentiality, while adhering to the Nexus Code of Conduct. Demonstrates self-awareness and willingness to process one’s own emotional and behavioral responses to situations in the workplace and demonstrates professional use of self in day-to-day work situations. Recognize and value cultural differences in all aspects of work and service delivery Required Education And Licensure Bachelor’s degree in Social Work preferred, Bachelors in Human Service Degree required Minimum 21 years of age Must successfully pass a DHS/DCFS/CJIS/DHHS/ODHS or other federal and state criminal background check Valid driver’s license required. Must meet state regulating agency driving requirements, maintain auto insurance and pass corporate insurance eligibility standards Must have the ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges. Must be open to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
