Licensed Vocational Nurse (LVN) - Home Health Assignment Duration This is a 13-week contract position. Schedule Monday to Friday, with weekend rotation and days off during the week to prevent overtime. Shifts are 8 hours per day. Job Description We are seeking a dedicated Licensed Vocational Nurse (LVN) for a home health position in San Marcos, CA. The ideal candidate will have recent home health experience and be familiar with HomeCare HomeBase (HCHB) systems. The position includes routine visits and requires the successful nurse to accumulate 32 points per week. Responsibilities Conduct routine home health visits to provide patient care. Maintain accurate and timely documentation of patient visits and care provided. Coordinate with healthcare teams to ensure comprehensive patient care. May be required to assist in other territories within the facility's service area, approximately a 60-mile radius. Required Experience / Certifications / Licensure Valid LVN license in the state of California. Recent home health experience is required. Experience with HomeCare HomeBase is preferred; candidates without HCHB experience may be considered at a reduced rate for initial training.
