Home Health RN Case Manager: Lead Patient Care at Home

AMEDISYS HOME HEALTH

Amedisys is seeking a full-time Registered Nurse Case Manager to assess patients, guide care plans, and collaborate with the care team to promote recovery at home in the Bangor area. You will supervise LPNs and HHAs, document promptly, and communicate progress to the clinical team. The role requires RN licensure in the assigned states, CPR certification, a valid driver's license, and reliable transportation.
