PRN RN Case Manager - Home Health Agency (or Contractor)

EMPLOYER Pediatric Nursing Specialists of Indiana (PNS), a Home Care Network (HCN) agency. JOB TITLE RN Case Manager – Home Health EMPLOYMENT TYPE All capacities available – Part‑Time, Full‑Time, PRN, Contractor, or Hybrid. SERVICE AREA Indianapolis, IN and surrounding 2‑hour radius. Primary service area centered on Indianapolis Metro, typical travel distances 10–60 minutes, occasional travel up to 2 hours. WORK SCHEDULE Primarily weekdays/daytime (Monday‑Friday, 8 am–5 pm). Flexible and customizable; no overnight or weekend‑only positions. No on‑call, overnight or overtime shifts required. Optional overtime and PRN services may be offered if desired. JOB SUMMARY As an RN Case Manager, you will coordinate patient care, manage and supervise a multidisciplinary care team, oversee assessments (OASIS & non‑OASIS), document care plans, discharge planning, and ensure optimal outcomes for a diverse client population. RESPONSIBILITIES Perform client assessments, direct, plan, implement and document appropriate actions. Supervise a team of RN, LPN, Home Health Aide and Personal Care Aides. Document, communicate and coordinate patient status and changes with the care team. Promote personal safety and maintain a safe environment for clients and coworkers. Travel to surrounding counties within service area. Ensure compliance with agency policies, HIPAA and quality standards. QUALIFICATIONS Graduate of an accredited School of Nursing. Active RN license in the state of Indiana, minimum 1 year of RN experience. Minimum 1 year in home health or acute care setting; 1 year of paid home health experience required. Minimum 1 year experience in case management. CPR and First Aid certification (current or to be obtained prior to hire). Experience in supervisory role, wound care, lab draws and OASIS assessments a plus. Strong background in nursing, patient care, confidentiality and documentation. Strong attendance and dependability record. Current driver’s license and reliable transportation. Familiarity with HIPAA regulations and medical terminology.
