Responsibilities
Conduct comprehensive assessments of residents' needs and develop individualized care plans in accordance with the Minimum Data Set (MDS) guidelines.
Collaborate with interdisciplinary teams to ensure seamless care coordination and delivery of high-quality services.
Monitor and report resident progress and concerns to family members, care providers, and other stakeholders.
Participate in quality improvement initiatives to enhance patient outcomes and drive continuous improvement.
Maintain accurate and detailed records of assessments, care plans, and resident interactions.

1- 2 years of experience in the field of MDS or related healthcare profession
Bachelor's degree in a related field (to be determined)
