PARAMEDIC - MIH

Overview Christian Hospital and Northwest HealthCare are seeking an experienced Mobile Integrated Health (MIH) paramedic to provide comprehensive post-hospital release follow-up care, conduct home physical exams, and collaborate with physicians to ensure continuity of care. The role includes monitoring medication, dressing changes, vital signs, and connecting patients to community resources. Preferred Qualifications Class (E) license preferred. 2 years of Paramedic experience required. Illinois paramedic license and IDPH MIH program completion. Role Purpose A Mobile Integrated Health (MIH) paramedic has a comprehensive medical knowledge base and is able to provide post-hospitalization care for a broad range of medical conditions. The MIH paramedic will also be able to provide advanced level medical care including post‑discharge evaluations, primary care in the home, and address social determinants of health. Responsibilities Conduct post-hospital release follow‑up care including monitoring medication, dressing changes, and checking vital signs. Conduct physical exams in patient's homes and work with physicians to provide continuity of care. Observe, record, and report patient conditions and reactions to drugs, treatments, and significant incidents to physicians. Work autonomously in the best interest of the patient. Conduct follow‑up visits to post‑discharged patients to provide necessary connections to community resources and primary care while monitoring medical needs.
