The job description is for a Home and Community Based Service Facilitator with JCOA. The position supports the Home and Community Based Service Director in coordinating programs such as Senior Centers, Elderly Nutrition, Care Management, Outreach, and Health Promotion/Disease Prevention.

Key responsibilities include:

• Processing program data and Louisiana Independent Living Assessments.
• Assisting with Care Management and Elderly Nutrition documentation.
• Monitoring nutrition program waitlists, meal routes, and participant rosters.
• Conducting quality assurance surveys for nutrition and in-home services.
• Supporting Site Managers and visiting senior centers to monitor operations.
• Assisting with quarterly Nutrition and Care Management meetings.
• Helping with meal delivery when needed.

The role has no supervisory responsibilities and is non-exempt, 35 hours per week . Preferred qualifications include experience in social services, casework, or working with older adults. The position requires strong communication, organization, attention to detail, independence, and the ability to manage multiple priorities. An associate degree is required , and one year of administrative experience is preferred. Proficiency in Microsoft Office, especially Word, Outlook, and Excel, is required.

The job is primarily office-based but may require travel to senior centers, client homes, and program locations. Physical requirements include walking, standing, bending, and lifting up to 20 pounds.
