Remote Care Manager - LTSS & Waiver Coordination
Molina Healthcare of Illinois
Molina Healthcare is seeking a field-based care coordination professional to support LTSS member care. You will collaborate with a multidisciplinary team to coordinate integrated services, including in-person home visits when required, and develop care plans with members, caregivers, physicians, and support networks.
Travel to local sites will be 25-40% as needed. You will monitor care plans, evaluate outcomes, promote service integration, and navigate waiver processes.
