Sentara Health Plans is seeking a Care Coordinator for LTSS Waiver Members/Medicaid, remote in Northern Virginia (Fairfax, Springfield, and Alexandria) and surrounding areas. This full-time role (40 hours) requires developing and monitoring multidisciplinary care plans for patients and families. Education must include a health/social sciences related degree from an accredited institution. 1 year LTSS/Medicaid/long-term care experience is required, with case management preferred. #J-18808-Ljbffr Sentara Health
