Bilingual Case Manager — Hybrid, Community Care & Advocacy

Sunnyside Community Services, Inc.

Sunnyside Community Services in New York is seeking a Bilingual Case Manager to help impaired older adults safely maintain independent life by linking them to community resources and home services, and advocating to obtain benefits. You will work with clients and families to formulate appropriate care plans, conduct home visits, and maintain contact by phone.
