Physical Therapist – CarePartners Home Health Franklin Team As a Physical Therapist, you play a vital role in helping patients regain strength, restore mobility, and rebuild confidence following illness, injury, or surgery. Working closely with physicians and the care team, you will assess each patient’s needs and create personalized therapy plans designed around meaningful goals, active engagement, and exceptional patient outcomes. Responsibilities Evaluate patient abilities and develop personalized treatment plans in collaboration with physicians and the care team. Deliver evidence‑based interventions that restore movement, reduce pain, and enhance quality of life. Monitor progress, refine therapy plans, and ensure accurate, timely documentation of outcomes. Educate patients and families on exercise programs, adaptive techniques, and safe practices that promote independence. Collaborate across disciplines to ensure cohesive, patient‑centered care and smooth transitions throughout recovery. Lead by example—mentor therapy assistants or students while upholding HCA Healthcare’s values of excellence, compassion, and integrity. Qualifications Basic Cardiac Life Support certification obtained within 30 days of employment start date. Licensed Physical Therapist obtained within 2 years of employment start date. Valid driver’s license. Bachelor’s degree. Minimum 1 year of experience.
