Schedule: MondayFriday, 8:30 AM5:00 PM EST (subject to business needs) Location: Hybrid (Syracuse, NY) 3 days onsite / 2 days remote following successful completion of onsite training We're looking for a Patient Referral Coordinator who wants their work to mean something someone who brings care, accountability, and purpose to what they do every day.

What You'll Do

In this role, you will:

• Ensure patient referrals are fully prepared for service by gathering required documentation, confirming insurance coverage, and securing authorizations in compliance with all regulatory requirements
• Coordinate new infusion and enteral referrals from intake through initial processing, ensuring accuracy, timeliness, and clarity at every step
• Collaborate closely with referral sources, hospital liaisons, pharmacy, nursing, billing, and branch teams to support seamless care delivery
• Communicate coverage details and financial obligations clearly to patients or their representatives, ensuring understanding and agreement prior to service
• Maintain accurate patient records, referral logs, and authorization tracking to support timely billing and reduce delays in reimbursement
• Identify opportunities to improve intake and referral workflows while maintaining high standards of service and compliance
• Bring reliability, follow?through, and thoughtful problem?solving to your daily work

Qualifications

• Associate's degree in Business, Healthcare Services, or a related field required
• Minimum of one (1) year of healthcare experience required
• Strong knowledge of payer requirements related to billing, authorizations, and coverage
• Proficiency with computer systems and documentation accuracy
• Strong organizational, multitasking, and problem?resolution skills

Preferred

• Two (2) or more years of experience with a home care or infusion provider
• Prior home infusion experience on the provider or payer side

Pay Range/Rate

$22 - $26 USD
