You will collaborate with the Nurse Manager, Director, Physicians, and staff to ensure high-quality, cost-effective patient care with strong safety and satisfaction outcomes. The position emphasizes leadership, staff supervision, and coordinated care planning for patients and families. Key Highlights Weekly pay: $1,807–$1,828 per week Guaranteed hours: 36 hours per week Contract duration: 13 weeks Shift: 3x12 Nights Start date: 08/03/2026 Setting: Home Healthcare - Hospice Responsibilities Provide unit-based leadership and coordinate care with the Nurse Manager, Director, physicians, and staff. Supervise and direct nursing and ancillary personnel; assign tasks and monitor performance. Initiate and coordinate conferences with health team members, patients, and families to develop or revise individualized care plans. Direct and monitor patient care activities to ensure safety, quality, and timely interventions; participate in patient care as needed. Foster patient- and family-centered care with attention to cultural diversity and language needs. Conduct rounds to assess patient needs, gather feedback on nursing services, and drive continuous quality improvement.
