About the Role

A comprehensive multidisciplinary rehabilitation organization is seeking a full-time Physical Therapist to provide individualized, evidence-based care in an outpatient rehabilitation setting. The therapist will evaluate patients, develop personalized treatment plans, implement therapeutic interventions, and collaborate with an interdisciplinary healthcare team to improve patient mobility, function, and overall quality of life.

Key Responsibilities

• Perform comprehensive physical therapy evaluations and assessments.

• Develop and implement individualized treatment plans.

• Deliver hands-on therapeutic interventions to support rehabilitation goals.

• Monitor patient progress and modify treatment plans as appropriate.

• Educate patients and caregivers on therapeutic exercises and home exercise programs.

• Collaborate with multidisciplinary healthcare professionals to coordinate patient care.

• Participate in interdisciplinary case reviews and rehabilitation planning.

• Maintain accurate, timely, and compliant clinical documentation.

• Adhere to professional standards, organizational policies, and regulatory requirements.

Required Qualifications

• Experience in physical therapy evaluation, rehabilitation, treatment planning, and therapeutic intervention.

• Strong patient care skills with the ability to educate patients and caregivers on home exercise programs.

• Knowledge of musculoskeletal, neurological, and orthopedic conditions.

• Current Physical Therapist license or eligibility for licensure in the applicable state.

• Doctor of Physical Therapy (DPT) or equivalent accredited degree.

• Excellent communication, interpersonal, and collaboration skills.

• Ability to manage a diverse caseload while maintaining accurate clinical documentation.

• Experience in outpatient rehabilitation is preferred.

• Experience working within specialty rehabilitation programs is considered an asset.
