Licensed Practical Nurse (LPN)

Position Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary care team. The LPN will provide direct nursing care in a long-term care and rehabilitation setting while supporting each resident’s physical, emotional, and social well-being.

The ideal candidate is dependable, works well with others, and is committed to delivering high-quality, resident-centered care.

Essential Responsibilities

• Administer medications and treatments in accordance with physician orders and facility policies.
• Assess residents, monitor changes in condition, and report concerns to the supervising RN or physician.
• Record vital signs and maintain accurate, timely clinical documentation.
• Assist with the development and implementation of individualized resident care plans.
• Collaborate with RNs, physicians, therapists, CNAs, and other healthcare professionals.
• Support residents’ rehabilitation and recovery goals.
• Provide residents and families with appropriate education and support.
• Supervise and support Certified Nursing Assistants and other direct care staff as assigned.
• Follow infection-control procedures and help maintain a safe, clean, and resident-focused environment.
• Comply with applicable state and federal regulations and facility policies.

Qualifications

• Current and unrestricted Licensed Practical Nurse license in the State of Florida.
• Current BLS/CPR certification.
• New graduates are encouraged to apply.
• Long-term care, rehabilitation, or geriatric nursing experience is preferred.
• Strong clinical skills, including medication administration and wound care.
• Excellent communication, documentation, and organizational skills.
• Ability to work effectively in a team-oriented environment.
