Job Details:

• Job Title: RN - Home Health
• Location: Jacksonville, FL
• Setting: Home Health
• Pay Range: $1800-$2000
• Schedule: 07:00:00 to 15:00:00
• Shift: Days, Monday-Friday, 5 shifts per week
• Contract Length: 13 weeks
• Candidate Type: Local and traveler friendly
• Requirements: Qualified applicants MUST have at least one year of experience in a Medicare-certified home health agency and be comfortable with OASIS (Outcome and Assessment Information Set), have a valid RN license and be willing to obtain FL licensure.

Responsibilities:

• Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
• Provide skilled nursing care to patients in their homes, including medication management, wound care, and infusion therapy.
• Perform OASIS assessments and documentation accurately and timely to ensure regulatory compliance.
• Educate patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence.
• Monitor patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes.
• Provide emotional support and counseling to patients and families coping with chronic illness or end-of-life care.
