Heart Failure CoordinatorThe Heart Failure Coordinator provides leadership and oversight of the heart failure program to ensure delivery of high-quality, evidence-based patient care across the continuum. This role coordinates multidisciplinary care, supports patient and family education, monitors quality and regulatory metrics, and leads initiatives to improve clinical outcomes and reduce hospital readmissions. The coordinator serves as a clinical resource for staff, collaborates with physicians and care teams, and drives program development, performance improvement, and accreditation readiness to achieve organizational goals related to heart failure care.Schedule:Full-time| 80 hours every two weeks | St. Cloud HospitalDay | 7am-4:30pm| Monday-FridayPay and Benefits:Pay based on experience and degreeStarting pay begins at $92,539.20 per year and increases with experience.Salary range: $92,539.20-$138,860.80 per year Salary range is based on a 1.0 FTE, reduced FTE will result in a prorated offer rateBenefits : medical, dental, PTO, retirement, employee discounts and more!Education:Graduate from a nursing programBSN or MSN preferredBSN or higher required within 36 months of being hiredExperience:5 years Minimum in nursing practice in ICU, Cardiology or Emergency Department settingLicenses and certifications:Registered Nurse (RN) Current licensure in the State of MinnesotaBasic Life Support (BLS) Current American Heart Association (AHA) Healthcare Provider card requiredAdditional qualifications:Highly developed analytical, critical thinking, and organizational skillsStrong verbal and written communication skillsBasic computer skillsCentraCare has made a commitment to diversity in its workforce.
