Occupational Therapist (OT) – Permanent

Location: Hayward, California
Compensation: $54–$59/hour
Schedule: 35 hours/week | Monday–Friday
Weekend Rotation: None
Facility Size: 99-bed Skilled Nursing Facility

Overview

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join a 99-bed skilled nursing facility in Hayward, CA . This permanent position offers a consistent Monday–Friday schedule with no weekend rotation , providing excellent work-life balance while allowing you to make a meaningful difference in the lives of your patients.

You'll have the opportunity to work with a diverse patient population, develop individualized treatment plans, and help patients regain independence in their daily activities. Responsibilities

• Conduct comprehensive occupational therapy evaluations and assessments

• Develop individualized treatment plans based on patient needs and goals

• Provide skilled OT interventions to improve functional independence and quality of life

• Address activities of daily living, mobility, coordination, strength, and other functional limitations

• Monitor patient progress and modify treatment plans as needed

• Accurately document evaluations, treatments, progress, and outcomes

• Collaborate with nursing, rehabilitation, and other members of the interdisciplinary team

• Educate patients, families, and caregivers on therapeutic techniques and strategies

• Participate in care planning and contribute to positive patient outcomes

• Maintain compliance with facility policies, clinical standards, and applicable regulations

Qualifications

• Graduate of an accredited Occupational Therapy program

• Current California Occupational Therapist license or eligibility for licensure

• Strong clinical, communication, and organizational skills

• Commitment to compassionate, patient-centered care

• Ability to work collaboratively with an interdisciplinary team

• SNF experience is preferred but not required

• New graduates are encouraged to apply
