Job Overview

We are seeking an experienced Advanced Registered Nurse Practitioner (ARNP) to join a patient-centered, community-based healthcare organization in Toledo, Washington. This role offers the opportunity to provide comprehensive primary care while collaborating with physicians, fellow ARNPs, Physician Assistants (PAs), and nursing staff.

Working within the Patient-Centered Medical Home (PCMH) model, the ARNP will deliver high-quality, evidence-based care to a diverse patient population, promote preventive health, and support improved patient outcomes.

Key Responsibilities

• Perform advanced clinical assessments and manage illnesses and medical conditions within the ARNP scope of practice.
• Collaborate with physicians, ARNPs, PAs, and nursing staff to coordinate patient care.
• Develop and implement individualized patient education plans to promote optimal health and wellness.
• Apply independent clinical judgment and evidence-based practices when making patient care decisions.
• Maintain timely and accurate clinical documentation, meeting the 48-hour documentation timeline at least 95% of the time.
• Complete fee slips with appropriate diagnosis and procedure codes.
• Meet productivity expectations of at least 3,400 patient encounters annually on a full-time-equivalent basis.
• Participate in daily care coordination huddles as part of the PCMH model.
• Identify at-risk patient populations and monitor population health outcomes.
• Empower patients through self-management education, treatment guidance, and discharge planning.
• Explain procedures and treatment plans to patients before implementation.
• Participate in quality management, peer review, and departmental practice development.
• Support the clinical education and professional development of other healthcare professionals, including student nurse practitioners.
• Complete required annual training, including HIPAA, Cultural Diversity, Bloodborne Pathogens (BBP), and Sexual Harassment Prevention.

Qualifications

• Graduate of an accredited Advanced Registered Nurse Practitioner (ARNP) program.
• Current Registered Nurse (RN) and ARNP licensure in the State of Washington.
• Current prescriptive authority from Washington State.
• Experience practicing in a broad primary care clinic setting.
• Strong knowledge of the nursing process, including assessment, planning, implementation, and evaluation.
• Ability to exercise sound clinical judgment and deliver evidence-based, patient-centered care.
• Strong communication, collaboration, documentation, and organizational skills.

Preferred Qualification:

• Experience performing minor surgical techniques.
