Occupational Therapist (OT)

Full-Time | Part-Time | PRN

We are seeking a licensed, compassionate, and patient-focused Occupational Therapist (OT) to join our rehabilitation team and provide high-quality, skilled therapy services to residents in accordance with physician orders and individualized care plans.

This is an excellent opportunity for an OT who is passionate about patient-centered care, clinical excellence, and helping residents regain function, increase independence, and improve their quality of life .

• Evaluate residents and develop individualized occupational therapy treatment plans based on clinical needs and physician orders.

• Provide skilled occupational therapy services in accordance with established care plans.

• Assess functional abilities and implement appropriate therapeutic interventions to promote independence.

• Monitor resident progress and adjust treatment approaches as clinically appropriate.

• Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.

• Collaborate with nursing, therapy, and interdisciplinary team members to support positive resident outcomes.

• Participate in care planning, utilization review meetings, interdisciplinary conferences, and other clinical meetings as assigned.

• Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and strategies that promote independence.

• Maintain established productivity, documentation, and quality standards.

• Support regulatory compliance, reimbursement requirements, and survey readiness.

• Promote resident safety, dignity, independence, and overall well-being.

• Graduate of an accredited Occupational Therapy program.

• Current and active state license as an Occupational Therapist.

• Long-term care or skilled nursing experience preferred.

• Strong clinical documentation and communication skills.

• Excellent interpersonal, organizational, and time-management skills.

• Ability to work effectively within an interdisciplinary healthcare team.

• Compassionate, patient-focused, and committed to providing high-quality care.

• Ability to stand, walk, bend, and assist residents throughout the workday.

• Ability to safely lift, transfer, and support residents during therapy activities.

• Ability to perform physical tasks associated with patient care and therapeutic treatment.

• Comfortable working in a clinical and long-term care environment.

• Ability to manage the physical and emotional demands of patient care.
